Clients being served through SWAP are eligible for services typically provided through DVR. Additionally, SWAP may also provide the following services:

Job development / placement

One year of follow-up
New Patterns of Service
SWAP is designed to provide a new pattern of service that will enhance existing transition services and to provide employment services to an underserved population of youth. Unique components include:
Year round community-based services;
Case management services; and
One year of follow-up services after successful employment.
Sava Program
Information and support needed to make informed decisions and take positive action.
Personal advocacy and assistance with accessing medical information and counseling resources.
At the request of the client, we can arrange for and be present at meetings with criminal justice professionals, businesses, etc.
Hospital Expense Assistance - Sarah E. Loveland Fund
Fund administered by Community Foundation for the purpose of financial assistance for hospital bills up to $1,000 at Poudre Valley Hospital. People receiving old age pension, direct County relief, or other assistance from the public may receive help from the fund.
